Tuition is accepted via Cash, Check, Cash App, Venmo and Pay Pal
Tuition is due the student’s first day of school upon arrival.
Payment plans are availible.
If the tuition is not received by the deadline there is a Penalty Fee.

Late Fee (Pick-Up): $2.00 per minute of tardiness

I️ give consent to observations and documentation to be taken via notes, photographs and/or video, of my child, monitoring his / her academic growth using First Curriculum as their enrichment education program. Observations and documentation can be used as a tool for marketing and to monitor the growing success of the School of Nirvana.

Student Health History

Dietary Restrictions/Allergies If your child has any dietary restrictions and/or allergies, please list them below: 1) Dietary Restrictions 2) Allergies: 3) Medications It is preferred that all medications are administered at home, however understanding that for some students that may not be option. Here are a few guidelines for if you MUST have medication administration. - Medicine must be in original prescription bottle with pharmacy label, this bottle must be solely for S.O.N. (if you explain to your pharmacist that your child is in school, they will comply with your second bottle request) - Medicine must be handed directly to the director of School of Nirvana. - Medicine administrations must be kept in a Medicine Log notebook authorizing S.O.N each day to administer the medicine. It must include: - Child’s Name and Date - Name of Medication - Amount and Time to be given (preferably 1pm Lunch Time) - Emergency contact information if any questions - Parent/Guardian Signature (Legibly Written) 4. DO NOT SEND MEDICINE CONCEALED IN FOODS, JUICES, ETC.
